1. Gaps in communications about patients between primary care practices and designated mental health agency are mitigated by a Co-Management Agreement.
2. Gaps in communications between OB and Addiction practice is being mitigated by having OB services provided in the Addiction practice. (More details)
3. SASH patients are registered into the hospital EMR and flagged. This supports ID of SASH participants when they are admitted or have an ED visit to foster communications of the discharge plan to SASH
